Our services
From high-tech skilled nursing to companionship and end-of-life care, every service is planned with your physician and delivered at home.
Skilled Nursing
Care begins with a Registered Nurse Case Manager who evaluates your needs and develops a plan of care in conjunction with your physician's orders. We can be contacted any time to answer questions.
High-tech nursing
IV / IVIG therapy, chemotherapy, wound care and wound vacs, ventilator care, G-tube care, and PleurX catheter drainage.
Case management
Skilled nursing visits for hypertension, diabetes, and COPD; all catheter care; skilled and non-skilled shift care for long-term and continuous-care patients.
Rehabilitation at home
Physical, occupational, speech, and respiratory therapists deliver rehabilitation and supportive services in the comfort of home.
Social & infusion services
Medical social services connect patients with community resources; our infusion team manages infusion therapy and pain management at home, with help sourcing DME and IV supplies.
Telemonitoring
Remote patient monitoring lets patients who are chronically ill, elderly, or vulnerable live independently while being monitored 24 hours a day by skilled nurses — for better outcomes and fewer re-hospitalizations.
Three minutes a day
A daily home assessment measures weight, heart rate, SpO2, blood pressure, and temperature. The device uses verbal prompts to walk the patient through collecting vitals at a set time each day — it takes about 3 minutes.
Reviewed by clinicians daily
Readings transmit to a central monitoring station where our clinical staff reviews the data every day. Alert parameters are set per patient — if a reading is out of range, our staff is notified and can intervene.
Shared with your doctor
Data can be trended in graphic or tabular form for review, and relayed to your physician and/or caregiver as requested.
Personal Care
Care Unlimited provides non-medical caregivers and companions to assist you or your family member with activities of daily living — while allowing them to remain in their own comfortable home environment.
How caregivers help
Our caregivers assist with the everyday activities that keep life at home safe and comfortable — and provide the companionship that keeps it warm. Support is arranged around your family's schedule, from a few hours a week to continuous care.

Hospice Services
When a life-limiting illness no longer responds to cure-oriented treatment, our team of hospice care specialists provides services designed to bring comfort and support to you and your loved ones.
Comfort and dignity, at home
Since 1995, we have upheld a simple truth: our nation's mothers, fathers, grandparents — and sometimes spouses and children — deserve comfort and dignity, in the place they want to be, surrounded by family. Our hospice team supports both the patient and the family through every stage.
Talking about hospice is hard. Our team is happy to speak with you or your loved one's physician about whether hospice is the right step — with no pressure and no obligation.
Referrals
All patients must have a physician directing care for an acute condition or an exacerbation of an existing condition. The first visit occurs within 24 hours of receiving the referral, or as requested by the patient or physician.

For physicians: care plan oversight & billing
Physicians overseeing complex care for Medicare home health patients can be reimbursed for certification, re-certification, and care plan oversight. When you refer to Care Unlimited, we mail you the plan of care (Form 485) for review and signature, send 485 updates every 60 days for re-certification, and provide a monthly list of discharged patients so your office knows how and when to bill.
